he can be simply borrowed, difficult work, or personal problems,
may be fatal illness...like in romantic movies ?

How often do you see him? He could be using the illness as a way to see someone else and get out of spending time with you. I would beware, red flags are going up for me on this one, call me paranoid or untrusting and that I may be but there's reason behind my madness. So leave it to me to expect the worst. ;)
